Consulting with a medical herbalist is a mutual commitment & it is useful to outline our respective commitments, as this helps us both proceed with clarity.
Registration
- If following our first contact we both wish to proceed a Client Registration Form will be sent from me to you. This enables the client to engage with the medical herbalist prior to the consultation.
Appointments
- If you need to move or cancel your appointment, please ring or email me to arrange.
- Clients who may benefit from herbal treatment will be provided with this after the initial consultation or it will be posted to them following further research by myself.
- Clients who receive herbal treatment will be followed up in 3-4 weeks & will receive another months’ worth of herbal treatment if needed.
Treatment
- Please provide complete information about your health at – or prior to – your appointment. This includes diagnoses made by other health professionals & list any medications you are currently taking. Omitting to provide this information can result in incorrect diagnosis &/or inappropriate or ineffective herbal treatment.
- The treatment I provide is likely to involve dietary & lifestyle advice as well as the prescription of medicinal herbs.
- Herbal remedies may take the form of capsules, tinctures, teas or topical applications.
Payment
- At consultation clients will be asked to pay consultation fee.
- If herbal treatment is prescribed at consultation payment will be required at this time. If herbal treatment is prescribed, but needs to be posted to client, an invoice will be generated & will need to be paid before receiving herbs.
Confidentiality
I may discuss your case anonymously with other health professionals to better support you.
All information shared by you is confidential: your client notes are anonymised & stored securely.

Type of information collected by Julie Yendall and the purposes for which it is held
I collect and process personal data. This is data that identifies you or can be used to identify or contact you, such as your name, address, email address, and phone number. It also includes sensitive data such as your medical information.
When contacting me, I store your full name, email, phone number and medical details for the sole purpose of communicating with you as it pertains to our patient / practitioner relationship, and in fulfilling my role as your chosen Medical Herbalist.
Storage of information and security measures
The personal data I collect can be both digital and paper. All paperwork is kept in a locked, secure location. All digital data is held on devices with appropriate security measures in place. All email communications are conducted using a mail service with end-to-end encryption. Appropriate security measures are in place to protect personal data from being accidentally lost or used or accessed in an unauthorised way.
Retention
It is a legal requirement that I keep adult patient records for 8 years after cessation of treatment or death.
In the case of children and young people records are retained until their 25th birthday, or 26th if the young person was 17 at the conclusion of treatment, or 8 years after death.
Your Rights
You have the right:
